PRE-K & KINDERGARTEN REGISTRATION
Registration for Pre-K students and those entering
kindergarten* in Erie's Public Schools will be held:


April 19, 20, 21, 26, 27 and 28, 2016
On April 19, 21, 27 and 28, registration will run from 8:00 a.m. - 3:00 p.m.
On April 20 and 26, registration will run from Noon to 7:00 p.m.

Information on Pre-K and Summer Kindergarten Readiness Program

Note: Students currently enrolled in the Erie's Public Schools Pre-K program at the Erie Family Center or in one of the district's elementary schools do not need to re-register for kindergarten. Your child is already registered.



Please review the information below before enrolling your child in kindergarten:

Children enrolling in Kindergarten must be five (5) years of age before September 1. Children who turn five (5) years of age in September may apply to the Pupil Services Department for testing and review. Determination as to enrollment will be made on a case-by-case basis.

Documents required for enrollment are:

[ ] Proof of age (for example: birth certificate, baptismal certificate, a valid passport). Photo ID of parent is also required.
    
[ ] Two proofs of residency (i.e., a deed, lease, current utility bill, or property tax bill)

[ ] Proof of immunization

    4 doses of tetanus (one dose on or after 4th birthday)
    4 doses of diphtheria (one dose on or after 4th birthday)
    3 doses of polio (one dose on or after 4th birthday)
    2 doses of measles
    2 dose of mumps
    1 dose of rubella (German Measles)
    3 doses of hepatitis B 2 doses of varicella (chickenpox) or history of disease

[ ] Upon enrolling, you will be asked to complete a registration form, home language survey, physical and dental forms, and emergency contact information.
