Restraints

	Restraint Reporting Guidelines1. Notify parent within 24 hours. 2. Notify building administrator & special education supervisor immediately. 3. Complete Incident Form & RISC Form. 4a. Obtain a signed waiver within 10 days. OR 4b. Hold IEP revision meeting and submit revised IEP pages. 5. Mail incident form, RISC form, waiver and, if applicable, revised IEP pages to your special education supervisor. Forms are linked below.
